Post Title: Rehabilitation Sensory Worker (Physical Disability)

Minimum Job Requirements:

Professional:

To offer a service of rehabilitation training/advice to visually impaired people within Manchester including an assessment of need, packages of care and the needs of the carers.

To provide professional advice and guidance to individuals, families/carers, external and internal service providers on assessment issues, equipment and adaptations.

Duties Assessments ? undertake a need led assessment involving the service user and/or their carer, to identify both the rehabilitation needs and also the potential of individuals with a visual impairment and their carers.

To deliver an effective and high quality rehabilitation and advisory service to sensory impaired people.

Provide rehabilitation including mobility and orientation training, communication, daily living skills and social/personal needs.

In partnership with the service user and/or carer to prepare a written care plan.

To advise on the rehabilitation of visual impaired people and the availability of all services for visually impaired people.

To liaise with health to manage and facilitate the blind and partially sighted registration process.

To monitor, review and reassess individual packages of support and rehabilitation.

To formulate and implement goals and plans in relation to direct work with service users and their carers, which may help them to manage their lives in the Community.

To maintain accurate and relevant records and prepare written reports ensuring adherence to the Department’s policy of confidentiality.

To provide relevant statistical information as required.

Professional:

DipHE in Rehavilitative Studies or equivalent qualification is essential.

Experience in one or more of the following:

Visual Impairment Rehabilitation work

Working with and teaching blind and partially sighted people of all abilities, backgrounds and levels of emotional adjustment

Working in social care and management of social care teams.

An understanding of the main eye disorders, their functional implication and associated sensory, physical or mental impairments.

Knowledge of various teaching methods and techniques and when to apply in terms of rehabilitative work.

Skills to be able to communicate effectively with a wide range of people both orally and in writing and skills to work with people with disabilities and their families.

Familiarity with appropriate legislation, procedures and resources in order to ensure that the Social Services Statutory requirements are met.

The necessary personal skills to work with service users and their carers and to protect the rights of the individual whilst recognising conflicting interests.
